Burial for a Specified Time
($250.00 additional) This service will include: The service of the Funeral Director and personnel, administrative staff for securing and filing legal documents, removal of the decedent to the Funeral Home within a 20 mile radius, Funeral Home refrigeration, limited sanitary care of the decedent, and a proportionate share of overhead expenses and use of service utility van for transportation to the cemetery for burial. These services do not include: Casket, Obituary in local newspaper, cemetery charges or state death certificate fee. |

| FORWARDING
OF DECEDENT TO ANOTHER FUNERAL HOME (Without Formal Viewing and/or Rites or Ceremonies) |

| Our
charges include: Basic services of
Funeral Director and personnel, a
proportionate share of overhead costs,
local removal of decedent, embalming
and/or sanitary care of decedent,
minimum dressing, transportation to
commercial carrier (including
Portland), consulting with receiving
Funeral home (within North America),
securing of Death Certificate.
Does not include: Casket, shipping container for casket, a commercial carrier charges such as by air or by rail, cash advances or expenses of receiving Funeral Home, or the placement of obituary in local newspaper. Internal transfers require the additional cost for translation of legal documents required by Foreign Consulates, as well as charges for long distance telephone or fax services. There may be additional charges applied for utility van and mileage to obtain certification of these documents. These costs will be added to the account. |
